This form may be used by instructors to schedule Library Instruction (LI) sessions for their classes. The more tho 

rough you are in completing this form, the more likely that we will be able to accommodate your date and time request. Please apply for a session as far in advance as possible.

Please plan on attending and participating in the library instruction session with your students. When instructors attend and participate in library instruction sessions, the learning process for their students is greatly improved. If a research assignment is scheduled to follow the LI session, please provide a copy to the librarian scheduled to work with you prior to the session.
